What To Expect
Please arrive 15 minutes early to your first appointment to fill out your patient forms or you may fill them out on the patient portal before you arrive. Check in at the front desk. Our helpful staff will walk you through the process.
What To Bring
Insurance, valid ID, and any previous health records that you would like to discuss with the medical provider. If you require a doctor’s note, ask the medical assistant that escorts you to the exam room and they will prepare one for the doctor to sign.
Financing & Insurance
Insurances we accept - Select Health, Molina, Aetna, United Health Care, CIGNA, Blue Cross Regence, Medicare, Medicaid, Humana, EMI, Tricare west, Bright Health Care, DMBA, Wise Network.
Note, we do not accept - AARP, Healthy U, Health Choice Utah, OPTUM.
