The Site is not monitored in real time and is not a way to reach us in an emergency.

The content on this Site is provided for general informational purposes only and is not medical advice. It is not a substitute for professional diagnosis, treatment, or advice from a qualified healthcare provider. Never disregard or delay seeking professional medical advice because of something you read on the Site.

Using the Site, reading its content, submitting a form, or contacting us through it does not create a doctor–patient or other treatment relationship, and does not make you a patient of the clinic. A treatment relationship is established only through an in-person or telehealth appointment with one of our providers.

Every patient is different. Descriptions of programs, therapies, and services on the Site — and any testimonials or patient stories that may appear — reflect individual experiences and do not guarantee any particular outcome. Individual results vary.

The Site is intended for use by adults. By using the Site, you represent that you are at least 18 years old or are using the Site with the involvement of a parent or guardian.

By submitting your phone number through the Site and checking the applicable consent box, you agree that we may call or text you (including using automated technology) at the number provided to respond to your inquiry and, if you opt in, to send appointment reminders and related messages. Consent is not a condition of receiving any service. Message frequency varies; message and data rates may apply. Reply STOP to cancel or HELP for help. Scheduling, patient-portal access, and financing are provided through third-party services (for example, AdvancedMD and CareCredit). Your use of those services is governed by their own terms and privacy policies. We are not responsible for the content, availability, or practices of third-party sites and services linked from the Site. Submitting a scheduling request does not guarantee an appointment until we confirm it.

The Site and its content — including text, graphics, logos, and images — are owned by or licensed to American Wellness and are protected by intellectual property laws. You may view and use the Site for personal, non-commercial purposes. You may not copy, reproduce, distribute, or create derivative works from our content without our written permission.

You agree not to use the Site to violate any law, infringe others’ rights, submit false or misleading information, impersonate another person, transmit harmful or malicious code, scrape or harvest data, or interfere with the Site’s operation or security.

The Site is provided “as is” and “as available,” without warranties of any kind, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of harmful components, or that its content is accurate, complete, or current.

To the fullest extent permitted by law, American Wellness and its owners, providers, and staff will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of, or inability to use, the Site. Nothing in these Terms limits or affects any rights you may have as a patient under applicable law, or excludes liability that cannot be excluded under applicable law. Some jurisdictions do not allow the exclusion of certain warranties or damages, so some of the above limitations may not apply to you.

You agree to indemnify and hold American Wellness harmless from claims arising out of your violation of these Terms or your misuse of the Site.

These Terms are governed by the laws of the State of Utah, without regard to its conflict-of-laws rules. Any dispute relating to the Site will be brought in the state or federal courts located in Salt Lake County, Utah.

If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect.

We may update these Terms from time to time. Changes take effect when we post the updated Terms with a new effective date. Your continued use of the Site means you accept the updated Terms.
