Privacy Policy

American Wellness and Rehab Clinic (“American Wellness,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains how we collect, use, and share information when you visit wellnessandrehabclinic.com (the “Site”) or communicate with us by phone, email, or text message.

This policy covers our website and marketing communications only. It is separate from our Notice of Privacy Practices (HIPAA), which describes how the clinic handles your protected health information as a patient. If you are a patient, a copy of our Notice of Privacy Practices is available at our clinic and on request by phone or email using the contact details below.

Confidentiality of substance use disorder information

Records relating to substance use disorder treatment receive additional protection under federal law (42 C.F.R. Part 2). This website policy does not apply to, and this website does not collect, treatment records. Visiting the Site or submitting a contact form does not make you a patient and does not create a treatment record.

Please do not send medical information through this Site

Our contact form and email are not secure or HIPAA-protected channels. Please do not include medical history, diagnoses, or other sensitive health details when you contact us through the Site. To discuss your care, call us or use our patient portal.

Information we collect

Information you provide. When you use our contact form, request a callback, or email us, we collect the information you choose to give us — typically your name, email address, phone number, the service you’re interested in, and your message. Where our forms include a marketing-consent checkbox, we also record whether you gave that consent.

Information collected automatically. When you visit the Site, we and our analytics providers automatically collect certain technical information, such as your IP address, device and browser type, pages viewed, referring pages, and the dates and times of your visits. This is collected using cookies and similar technologies (see “Cookies” below).

Analytics and advertising technologies

We use third-party cookies and tracking technologies for quality-assurance purposes — to understand how visitors use the Site, verify that our pages and forms work correctly, and measure the performance of our advertising:

  • Google Analytics and Google Ads help us measure traffic, page usage, and ad performance. Information is processed by Google; see the Google Privacy Policy. You can opt out using the Google Analytics Opt-out Browser Add-on.
  • Meta (Facebook) Pixel and Conversions API are used on our marketing pages to measure our advertising and for retargeting — showing follow-up ads on Facebook and Instagram to people who have visited our marketing pages or contacted us. To do this, information about your visit (such as the page viewed and your device information) is shared with Meta, and we may share limited contact identifiers (such as an email address or phone number) in hashed (scrambled) form, which Meta uses only to match against its own accounts for advertising purposes. See the Meta Privacy Policy. You can review and limit how ads are targeted to you in your Facebook/Instagram ad settings.

These providers may combine the information they collect with data they hold from other sources. We do not control those practices.

Advertising technologies and health information

We use these technologies in a manner designed to comply with HIPAA, the federal confidentiality rules for substance use disorder information, and the advertising platforms’ healthcare policies:

  • Advertising tags run only on our general marketing pages. We do not run advertising pixels on our patient portal, scheduling pages, or any page used to provide care.
  • We never send diagnoses, health conditions, treatment information, medical history, or the content of your messages to any advertising platform.
  • Where we share contact identifiers with Meta for ad matching, we share them only in hashed form, only for people who have consented to marketing communications, and only to measure and deliver our own advertising — never for the platform’s or anyone else’s marketing.

Please be aware that visiting a healthcare website can itself signal an interest in the services described on it. If you prefer that no advertising platform receive information about your visit, use the opt-out tools described in “Your privacy choices and rights” below, or browse in a private/incognito window with third-party cookies blocked.

Third-party services and links

The Site links to and integrates services operated by others. When you use them, their terms and privacy policies apply, not ours:

  • AdvancedMD — online scheduling and patient portal.
  • CareCredit — financing information and applications.
  • YouTube (embedded video) and Google Maps (embedded map) — provided by Google, which may set cookies and receive your IP address when these load.

How we use information

We use the information we collect to:

  • respond to your inquiries and requests, including calling or texting you back when you ask us to;
  • send you information about our services if you have asked for it or consented to marketing communications (you can opt out at any time);
  • operate, maintain, and improve the Site;
  • measure site traffic, usage, and advertising performance; and
  • comply with legal obligations.

Blog comments and article feedback

Our blog lets you leave a comment on an article and tell us whether an article was helpful. Both are optional, and you can read everything on the Site without using either.

If you leave a comment, we collect the display name you choose and the text you write. We do not ask for your email address, and we do not ask whether you are a patient. We also record a one-way scrambled version of your IP address, which we use only to limit automated spam and which we delete after 30 days.

  • Comments are reviewed by our staff before they appear, and are public on the Site once approved. Please do not include medical information in a comment. If you need to discuss your care, call the clinic or use our patient portal.
  • We do not publicly confirm whether a person who comments is or has been a patient of the clinic.
  • Comments that are not approved are deleted after 30 days.
  • You can ask us to remove your comment at any time using the contact details below.
  • The “Was this article helpful?” buttons record only a running total of yes and no responses for that article. We do not record who voted, and the totals are not shown publicly.
  • We keep a count of how many times each article is viewed. This is a total only — it is not linked to you, your device, or your other activity on the Site.

Comments are stored on our own systems. We do not use a third-party comment service, and no advertising or tracking technology is loaded by the comment or feedback features.

Text messages (SMS)

If you provide your phone number and consent to receive text messages, we may text you to respond to your inquiry, confirm or remind you about appointments, or follow up on your request. Message frequency varies; message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. Consent to receive text messages is not a condition of receiving any service. We do not share your mobile number or SMS consent with third parties for their own marketing.

How we share information

We do not sell your personal information, and we do not share it with third parties for their own marketing. We share information only with:

  • service providers who help us operate the Site and communicate with you (for example, our website, customer-relationship-management, email, and text messaging platforms, and the analytics providers listed above), who may use it only to provide services to us;
  • advertising and analytics providers through the cookies and tags described above — some state laws treat this as “sharing” or “targeted advertising,” and you may opt out as described in “Your privacy choices and rights”; and
  • others when required by law, or to protect our rights, safety, or property, or in connection with a sale or reorganization of our business.

Cookies

Cookies are small files stored on your device. We use them to run the Site and, with the analytics and advertising tools above, to understand usage and measure ads. You can control cookies through your browser settings; disabling some cookies may affect how the Site works.

Your privacy choices and rights

  • Opt out of targeted advertising cookies: use your browser’s cookie controls, the Google Analytics opt-out add-on linked above, Meta’s ad settings, or industry tools such as the Digital Advertising Alliance opt-out page.
  • Opt out of retargeting: you can ask us to exclude your contact information from our advertising audiences at any time by contacting us using the details below.
  • Opt out of marketing email: use the unsubscribe link in any marketing email we send.
  • Opt out of text messages: reply STOP to any message.
  • Access, correction, and deletion: you may ask us what personal information we hold about you from your use of the Site, and ask us to correct or delete it, by contacting us using the details below. Depending on where you live, state law may give you additional rights, including the right to opt out of targeted advertising; we will honor requests as required by the law that applies to you, and we will not discriminate against you for exercising your rights. (Requests concerning your medical records are handled under our Notice of Privacy Practices, not this policy.)
  • Do Not Track: some browsers offer a “Do Not Track” signal. Because there is no common industry standard for responding to it, the Site does not currently respond to Do Not Track signals.

Data retention

We keep the information you send us for as long as needed to respond to you and for our legitimate business and legal purposes, then delete or de-identify it.

Children’s privacy

The Site is intended for adults and is not directed to anyone under 18. We do not knowingly collect personal information from children under 13; if you believe a child has provided us personal information, contact us and we will delete it.

Security

We use reasonable administrative, technical, and physical measures to protect information collected through the Site. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.

Changes to this policy

We may update this Privacy Policy from time to time. Changes take effect when we post the updated policy with a new effective date. If we make material changes to how we use information you have already given us, we will take reasonable steps to notify you.
